For questions, concerns, or clarification please email CONCERNS@200PERCENTSPORTS.COM. ********COVID-19 AWARENESS AND WAIVER AGREEMENT: By signing this entire agreement you have agreed to review all considerations for youth sports, listed on the CDC.GOV site link below this paragraph. Please click on the link and review all risks associated with COVID-19 and youth sports interactions. This waiver agreement, upon signing, will relieve 200 Percent Sports Inc (and the event hosts associated with any event attended) from any liabilities due to possible contact with Covid-19 infected person(s). You understand that this is your decision to allow your player to participate in youth sports with the risks listed and explained in the link below and agree to your child wearing a protective mask when they are not exercising: htts://www.cdc.gov/coronavirus/2019-ncov/community/schools-childcare/youth-sports.html The Assumption of Risks, Medical Release, and Nonrefundable Fees Policy This form has four parts (1) an assumption of risk and release; (2) paragraph of instruction and agreement (3) medical authorization and (4) Nonrefundable Fees Policy. The first section tells you about risks of injury that may arise from participating in a sport or physical activity of this youth sports program/tournament, in order to aid you in making an informed decision as to whether or not your child should participate in this athletic activity, and requires you to assume risk and responsibility. The second section emphasizes obedience to all safety rules and regulations by you and your child. The third section gives 200% Sports Inc. authorization to provide medical care in the event of an accident; should you happen to not be reached via all provided contact information. The fourth reviews and binds members to the nonrefundable fees policy of 200% Sports. As a parent or guardian, you should ask coaches, physicians, and other knowledgeable persons about any concerns that you might have at any time about your child’s participation and safety. The decision for your child to participate is yours alone. If you feel pressured by any outside party, we advise you to take a pause and make an educated and well thought out decision. I. Assumption of Risk Injuries to participants in youth sports programs and/or tournament play may occur from risk inherent in the sports or activity; from placing stress on the body that has not been prepared for; from accidents in learning or practicing, from failing to follow game, training, safety or other team rules; from the use of transportation to and from events or practices provided by our organization at your request, and from “Good Samaritan” administration of first aid. Injury can include direct physical, and possibly crippling, injury to one’s body, and emotional injury experienced as a result of inflicting injury to another or witnessing it. Emotional injury can also be caused if a child is ill-prepared to process the discipline and authority involved in playing youth sports. The severity of the physical injury can range from minor cuts, scrapes, or muscle strain to catastrophic injuries, such as paralysis or even death. In consideration of 200% Sports Inc. permitting my child or ward to participate in its youth sports program, I hereby agree on behalf of my child that he or she will assume the risk of injury or death from participating in the above as outlined. I release 200% Sports Inc. and it’s board members, volunteers, coaching staff, spectators and participants from any liability resulting from my child’s participating in sports or activities. This assumption of risk and release binds my child’s heirs, estate, executor or administrator, and assigns all members of my family. II. Instruction and Agreement I have instructed my child to obey all directions of the instructors and personnel in charge of the sports or activity and their assistants and volunteers; to comply with all safety instructions; and to refrain from horseplay and any other unsafe practices, to include using profanity or aggressive behavior towards other children or adults. I understand that as a parent/guardian, I am responsible for my behavior when interacting or spectating in practices and sporting events and activities. I understand that I have a right to request a hearing with the 200% Sports Inc. board and committees should I have any grievances or complaints about the product I am receiving or the conduct of any representatives of 200% Sports Inc. or its affiliates. I understand that 200% Sports Inc. has fourteen days to schedule my hearing upon written request. I am aware that I am not to engage in unsportsmanlike conduct, use profanity, use illegal drugs, alcohol, marijuana (to include spectating while under the influence) while around any activities or children under the care of 200% Sports Inc. or its events. I am aware that 200% Sports Inc. reserves the right to sever our agreement immediately, should my, or my child’s conduct or behavior be deemed inappropriate by the 200% Sports Inc. board members. Under these circumstances, I will not receive a refund of any kind. I will abide by their requests if I am deemed an unhealthy presence at practices, events, or activities, without rights of recourse of any kind. III. Medical Authorization In the event of an accident or illness, I authorize 200% Sports Inc. to provide medical treatment for my child if I cannot be contacted immediately and I hereby consent to the administration of any and all medical procedures deemed necessary by the attending authorities. I understand that 200% Sports Inc., it’s board members, staff, volunteers, and spectators assume no financial obligation or liability for the immediate medical treatment that they provide for my child. IV. Nonrefundable Fees Policy All deposits, equipment fees, and registration money are nonrefundable, even in the event that seasons or tournaments are cancelled or rescheduled. In the event of an injury/illness that prevents a player from participating, a doctor's note is required and must be verified. In this event, a partial refund for registration funds only may be refundable pending a review with the 200% Sports Inc. board members. 200 Percent Sports Inc has 30 days to review and make a decision on all refund requests. A refund may take 60-90 days to issue. Training may be given in lieu of a refund. If there are any questions or concerns about refunds or cancellations, please call 702-306-5552 for clarifications. Refunds will also not be given for lack of playing time. This is not a "pay to play" organization.
